As you move from hospital to home the Irish Heart Foundation is here to support you. We work with many health professionals including cardiac nurses to give you information, advice, and support on living with your heart condition.
Nurse Support Line
Irish Heart Foundation nurses are available to answer all your questions and concerns, whether you have specific queries, worries, or just want to talk about your condition. You can speak with one of our nurses by calling 01 6685001 or emailing support@irishheart.ie.
Heart Support Network
Join our Heart Support Network private Facebook group, where you can find expert information from cardiac nurses, doctors, and other health professionals. It is a place to link in with others living with similar heart conditions, see how they are coping and share thoughts and experiences. We have weekly physical activity sessions, regular live chats with health professionals and counsellors as well as a variety of videos on health topics and practical advice. You can find our group by logging into your personal Facebook account and searching for “Heart Support Network Irish Heart Foundation”.
Cardiac Rehab for All
Cardiac Rehab for All is a programme of physical activity and education for people who have had a heart attack or other types of heart events and conditions. Created by the Irish Heart Foundation and the Irish Association of Cardiac Rehabilitation, it can be used at home or as part of your hospital based rehabilitation. It can help you and your you learn to live with a heart condition. It has a wide variety of information and advice about your heart health. Heart Failure Support
Join one of our monthly heart failure support meetings, where you will meet expert speakers on topics such as self-management, medications, exercise, diet and mental health. It is also a place for you to meet others who are on a similar journey to you as you navigate your way through your diagnosis and treatment. For further information email heartfailure@irishheart.ie.
Fluid Heart Tracker
This is a free app to help people living with heart failure with their daily
weight monitoring. Simply enter your weight every day into the app. If your
weight increases significantly, it may mean you are retaining fluid and the
app will alert you and advise you to contact your doctor or nurse. This app
may help you to manage some of your heart failure symptoms, but it is
important to remember that it is not a substitute for medical advice. If you
